Caring Connection is a program of the National Hospice and Palliative Care Organization, a consumer and community organization committed to improving care at the end of life.

The Five Wishes document helps people express  how they want to be treated if they are seriously ill and unable to speak for themselves .  It includes medical, personal, emotional and spiritual needs.

The Texas Health and Human Services Commission Quality Monitoring Program web-page is a resource for evidence-based best practice to help providers do the right thing in the right way at the right time to achieve the best possible outcome.
